Info Sheet: It is required for ALL our families to fill out the Info Sheet as a volunteer or non-volunteer.
Volunteering: Those opting out of the volunteer non-participation fee are agreeing to volunteer 4-6 hours this semester. Individuals who do not complete volunteer duties for the semester (or work out an alternative with our coordinator) will be assessed the $125 fee at the end of the semester.
Photo Release: I am aware that individual and group publicity photos and videos are taken, and I hereby grant permission for my student’s likeness to be used in CCC’s publicity or advertising.
Cancellations/Refunds: I understand that a $75 deposit/handling fee for training choirs ($100 for performing choirs) will be withheld upon student cancellation for new families. Cancellations must be received in writing (email) within 72 hours of the second class. (Continuing students that withdraw will not be refunded once classes begin.) Tuition will be acknowledged as a charitable donation.
Partial Payment Fee: I am aware that, should I choose to pay the semester's tuition in installments, a $20 administrative fee will be added to my account balance. N/A for the whole year registration.
Parent Handbook: I have read, understand and agree to the terms outlined in the parent handbook.
