Photo Release: I am aware that individual and group publicity photos and videos are taken, and I hereby grant permission for my student’s likeness to be used in CCC’s publicity or advertising.
Cancellations/Refunds: I understand that a $75 deposit/handling fee for training choirs ($100 for performing choirs) will be withheld upon student cancellation for new families. Cancellations must be received in writing (email) within 72 hours of the second class. (Continuing students that withdraw will not be refunded once classes begin.) Tuition will be acknowledged as a charitable donation.
Partial Payment Fee: I am aware that, should I choose to pay the semester's tuition in installments, a $20 administrative fee will be added to my account balance. N/A for the whole year registration.
Parent Handbook: I have read, understand and agree to the terms outlined in the parent handbook.
